Ex-employees can submit request for refund on ground of Medical
exigencies by logging/Registering into the portal.
1. Ex-employees who have already Registered/submitted data on the Amicus portal can login to
this portal by entering mail-id as login-id and phone number as password. Once logged-in the
homebuyers have to update following fields in the form:
- Does ex-employee suffer from any Serious Medical Condition
- Describe the Nature of the serious Medical Condition
- Supporting Documents From Hospital/Institution
- Name of Hospital/Doctor treating
- Date of Medical prescriptions (Latest) uploaded on portal
- Expenses incurred for the medical treatment from 01.01.2021 onwards
- Have the medical expenses been re-imbursed by the employer or claimed by him under any
Medi-claim policy
Ex-employees will be able to update fields related with medical exigency. Do not forget to click on Update button below and save the updated
data.
2. If you have not registered earlier on Amicus Portal you can register now by clicking on Register Button on the Home Page. Fill in all required fields in the form and save the information by clicking on Register button on Registration Form.
